• If applicant is govt. servant then NOC by head of department
• Proof of Age(school leaving certificate or certificate issued by Civil surgeon
• Proof of address (Any one from Tax bill of nagarpalika/gram panchayat, Light bill, Telephone bill, EPIC card,Diving license)
• The evidence for physical risk
• The evidence for financial risk
• Certified copy of ration card.
